Un aggiornamento su tutte le tecniche più diffuse al momento in Italia con l'intervista alla nostra esperta, la Dottoressa Simona Blidar, papessa della medicina estetica a Milano.
Che cosa significa oggi fare Medicina Estetica?
“I migliori risultati sono quelli che non si ‘vedono’, che esaltano l’armonia di ogni viso, la freschezza e la naturalezza della pelle nuda, compensando quello che gli anni “tolgono”, ma senza mai esagerare. Utilizzo i migliori prodotti ed apparecchiature, senza fare compromessi che, con il tempo, lascerebbero il segno di ‘viso rifatto’.”

Che cosa chiede oggi il paziente e quali sono i prodotti preferiti dai medici?
“La follia delle iniezioni continua nel 2021: Botulino, acido ialuronico, cocktail biorivitalizzanti e biostimolanti potentissimi e i recentissimi enzimi. Oggi, i pazienti ci chiedono soprattutto risultati naturali, visibili solo ai loro occhi. L’acido ialuronico, una delle scoperte più belle dello scorso secolo, ha superato già da tempo il ruolo piuttosto limitato di riempitivo di rughe. Con le nuove tecniche avanzate si può agire su tutte le parti anatomiche che subiscono l’invecchiamento: strutture ossee, legamenti, compartimenti di grasso e infine pelle, ma anche correggere asimmetrie oppure disarmonie tra i vari elementi facciali (come una linea mandibolare poco marcata, il gibbo nasale oppure un mento sfuggente).”
La paura del Botulino o dei suoi effetti frozen-face è ancora giustificata?
“Sono, oramai, ben conosciuti gli effetti benefici delle iniezioni con la proteina botulinica di tipo A (frequentemente nominata Botox®) e molti miei pazienti effettuano il trattamento all’insaputa di amici o familiari – perché, se eseguito bene, risulta molto naturale. Ci sono comunque alcuni aspetti di critica importanza da valutare prima di effettuare questo efficace trattamento:
- primo, il prodotto utilizzato deve essere uno dei tre approvati dalle autorità italiane
- secondo, le iniezioni con il Botulino devono essere amministrate da un medico con esperienza e in un ambiente idoneo, cioè l’ambulatorio medico.
“Per una maggiore naturalezza, le tecniche del ‘Botox’ si stanno evolvendo insieme alla nostra esperienza. Un’analisi approfondita dell’anatomia (ampiezza e forza di ogni muscolo) e della mimica facciale valutata individualmente, ci indicano la giusta diluzione e i punti precisi di iniezione per un risultato bilanciato, che mantiene la naturalezza e l’espressività faciale.”

Nomini un trattamento di ultima generazione:
“La biostimolazione enzimatica è la NOVITÀ che sta rivoluzionando l’approccio terapeutico nei processi fibrotici – come le cicatrici ipertrofiche e i cheloidi e nella riparazione del collagene nell’antiaging! Si tratta di prodotti altamente purificati in una soluzione di acido ialuronico ed enzimi (collagenasi, lipasi oppure liasi):
- la COLLAGENASI Col/GH PB220 distrugge le pareti di collagene presenti nei noduli fibrotici della cellulite e degrada ed elimina le fibre di collagene amorfe, disorganizzate e disfunzionali presenti nel derma del tessuto fibrotico e cicatriziale,
- la LIPASI Lipase PB500 stimola il metabolismo dei lipidi e la ripartizione dei trigliceridi. Riduce il volume degli adipociti,

Le principali indicazioni della biostimolazione enzimatica sono:
- trattamento e prevenzione della fibrosi post-chirurgica,
- cicatrici chirurgiche, da traumi o da acne,
- strie distensae (smagliature),
- PEFS III e IV stadio (cellulite avanzata),
- fibrosi con accumulo di grasso,
- doppio mento,
- edema associata a fibrosi,
- borse perioculari.
Esiste un trattamento antico, ma allo stesso tempo moderno?
“Sì, il PEELING…… il trattamento estetico più antico della storia…… I peeling chimici sono stati progettati per ridurre e possibilmente invertire l’invecchiamento della cute (perdita di elasticità, macchie, rughe, cheratosi attiniche, etc.). Attualmente abbiamo a disposizione varie formulazioni al cui scopo principale è di indurre un turn-over (desquamazione + rigenerazione) della pelle; alcune soluzioni sono molto potenti, capaci di ‘liquefare’ quasi l’intero spessore del derma, altre più dermo-stimolanti e meno distruttive. Più profondo è il peeling, migliore sarà il risultato. I peeling medici devono essere eseguiti solo da un medico con una corretta formazione. Il risultato? Pelle radiante, levigata e una forte stimolazione della sintesi del collagene: linee sottili, macchie, cicatrici e occhiaie si riducono in un mese.”

“ Il D.A.S. Medical – sistema ablativo superselettivo al plasma che attraverso il principio della radiofrequenza a onde lunghe che genera plasma crea una sublimazione dei tessuti selettiva e delicata. Trova un grande utilizzo nel trattamento degli inestetismi cutanei benigni, cicatrici o blefaroplastica non-chirurgica e cheiloplastica (le rughe codice a barre) non-chirurgica.”

I principali vantaggi di questa tecnica:
- il needling crea delle micro-perforazioni nelle cicatrici o nelle rughe e stimola il processo di riparazione tessutale e la produzione di nuovo collagene ed elastina,
- la penetrazione ‘pixel’ del TCA crea meno danni chimici all’epidermide, con una forte stimolazione cutanea (senza i rischi del peeling profondo) e meno disagio per il paziente.

An update on all the most popular techniques currently available in Italy with an interview with our expert, Dr. Simona Blidar, the godmother of aesthetic medicine in Milan.
What does it mean today to do Aesthetic Medicine? "The best results are those that you do not "see", which enhance the harmony of each face, the freshness and naturalness of bare skin, compensating for what the years "take away", but without ever exaggerating. I use the best products and equipment, without making compromises that, over time, would leave the mark of a 'redone face'."
What does the patient ask for today and what are the products preferred by doctors?
"The insanity of injections continues in 2021: Botox, hyaluronic acid, ultra-powerful bio-revitalizing and bio-stimulating cocktails, and the latest enzymes. Today, patients are mostly asking us for natural results, visible only to their eyes. Hyaluronic acid, one of the most beautiful discoveries of the last century, has long outlived its rather limited role as a wrinkle filler. With the new advanced techniques we can act on all anatomical parts that suffer aging: bone structures, ligaments, fat compartments and finally skin, but also correct asymmetries or disharmonies between the various facial elements (such as a poorly marked mandibular line, nasal hump or a receding chin)."
Is the fear of Botulinum or its frozen-face effects still justified?
"The beneficial effects of injections with botulinum protein type A (frequently referred to as Botox®) are, by now, well known, and many of my patients perform the treatment without the knowledge of friends or family - because, if performed well, it looks very natural. However, there are a few critically important aspects to evaluate before performing this effective treatment:
- first, the product used must be one of three approved by the Italian authorities,
- second, Botulinum injections must be administered by an experienced physician and in a suitable environment, i.e., the doctor's office.
"For more naturalness, ‘Botox’ techniques are evolving along with our experience. An in-depth analysis of anatomy (amplitude and strength of each muscle) and individually assessed facial expressions tell us the right dilution and precise injection points for a balanced result that maintains naturalness and facial expressiveness."
Nomini a state-of-the-art treatment:
"Enzymatic biostimulation is the NEW thing that is revolutionizing the therapeutic approach in fibrotic processes - such as hypertrophic scars and keloids and in collagen repair in anti-aging! These are highly purified products in a solution of hyaluronic acid and enzymes (collagenase, lipase or lyase):
- COLLAGENASE Col/GH PB220 destroys the collagen walls present in fibrotic cellulite nodules and degrades and eliminates amorphous, disorganized and dysfunctional collagen fibers present in the dermis of fibrotic and scar tissue,

Is there such a thing as an ancient yet modern treatment?
"Yes, PEELING...... the oldest aesthetic treatment in history....... Chemical peels were designed to reduce and possibly reverse skin aging (loss of elasticity, spots, wrinkles, actinic keratoses, etc.). Currently we have available various formulations whose main purpose is to induce a turn-over (desquamation + regeneration) of the skin; some solutions are very powerful, capable of "liquefying" almost the entire thickness of the dermis, others more dermo-stimulating and less destructive. The deeper the peeling, the better the result. Medical peels should only be performed by a physician with proper training. The result? Radiant, smooth skin and a strong stimulation of collagen synthesis: fine lines, blemishes, scars and dark circles are reduced in a month."

- Needling creates micro-perforations in scars or wrinkles and stimulates the tissue repair process and the production of new collagen and elastin,
- the "pixel" penetration of TCA creates less chemical damage to the epidermis, with a strong skin stimulation (without the risks of deep peeling) and less discomfort for the patient.
